Health Inequalities Intervention ToolkitThis toolkit is designed to support Primary Care Trusts with their Local Delivery Planning and commissioning

It builds on the previous Health Inequalities Intervention tool by providing a spearhead tool to monitor life expectancy gaps, an all area tool for life expectancy monitoring across all English local authorities and an infant mortality tool to assess the impact of modifying risk factors contributing to higher rates of infant deaths.
